2008-2009 Parent/Child Policies, Registration Form, and Release of Liability Waiver. Please complete, sign and date all three forms and return them to the YMCA.
Please include the following specific information on the registration form: Child's Name, Program Name, Start date, Fees. Reference the example on the downloaded registration form.
Participation in the YMCA Y-Guide/Princess program requires that
both the parent and the child maintain a Community Membership
throughout duration of the program (October 2008 - September 2009).
Fees include an annual $90 per family membership fee and an annual
$60 per individual
program fee.
